Silver Chain staff located on the Sunshine Coast are aware of their pioneering role in establishing a local Silver Chain presence in Queensland’s evolving health environment. Silver Chain’s out of hospital care offered on the Sunshine Coast evidences better options for client-centred care. Targeting this essential “right” has resulted in a greater than 63% death rate at home since January 2013 for referred palliative clients.
Silver Chain Queensland’s specialist community Palliative Care Service assists clients across the Sunshine Coast area. The team works in close collaboration with other primary practitioners, allied health teams and regional palliative care services.
The service includes:
• Assessment and care planning for the client.
• Management of complex client care needs.
• Assisting clients and families with advanced care planning requirements.
• 24 hours a day, 7 days a week phone support service.
Our post-acute careservice provides early discharge support to Sunshine Coast clients who are leaving hospital. Silver Chain also provides nursing support to Transitional Care clients for Queensland Department of Health, with some domestic assistance, personal care and respite services.
