The 2014/15 financial year has set Silver Chain Queensland on course for a bright future.
It has been less than two years since we received Queensland Health funding to provide Hospital in the Home (HITH) services in Queensland.
HITH is a true hospital substitution model where those with acute conditions who would otherwise require hospital based care are transferred to Silver Chain to receive acute care in the comfort of their own home.
Over the past year we have continued to establish our HITH service in the Metro North and Sunshine Coast Hospital and Health Services (HHS) and in March 2015 we expanded our reach and impact with the commencement of our HITH service in the Mackay HHS.
In just a year we’ve seen real progress in achieving more admissions to our services.
We have been working closely with our stakeholders in each HHS to ensure our HITH service meet their local needs. We have aligned our service models, changed our workflows and improved communication and liaison.
Silver Chain’s approach to service delivery in Queensland is innovative and this is particularly evident in the Sunshine Coast region where we hold medical governance. To achieve this we have invested in General Practitioners, interested in providing acute care in their community.
We continue to receive positive feedback from our clients about our services through regular satisfaction surveys. Of those who responded to the survey during the past year, 70 per cent gave the highest rating of totally satisfied and 96 per cent of clients would not have preferred to have been admitted to hospital.
At the beginning of the year we emerged as a small team but with growth and expansion of our services, rapid establishment of our workforce was required. We now employ 50 staff and moved from serviced office sites to new bases in Banyo, Brisbane and Mooloolaba, Sunshine Coast in December 2014 and in Mackay Central in March 2015.
We have continued to maintain our quality and safety standards over the past year and spent most of the year preparing for EQuIP National standard accreditation in late November 2015.
