The past financial year for Silver Chain Group has been characterised by focus on vision and performance.
The past financial year has seen Silver Chain Group adapt and respond to the ever-changing operating environment across the health care and aged care sectors.
Social Care is a newly created division of Silver Chain Group, bringing all aged care services in both metropolitan Perth and regional Western Australia together into one team. This year, our dedicated staff helped more than 16,000 clients continue to live independently at home, through more than 1,200,000 hours of care.
Access Care Network Australia (ACNA) this year moved another step closer to achieving its goal of supporting 100,000 clients to access the support they need to live independently. We helped more than 53,000 older Australians, carers and people ready to be discharged from hospital receive support with activities of daily living.
Over the past year, Health Care has had a significant impact on the health and wellbeing of Australians, supporting almost 65,000 clients to live and die well in the community, through the provision of more than 1,700,000 hours of care.
Over the past financial year, we have continued to support, develop and motivate our 4,000 staff and 350 volunteers so they can continue to provide the high standard of service to our clients that we have become renowned for in the community.
During the past year, the Information Services team has continued to provide support to both clients and staff through innovative technological solutions.
The past year has been one of consolidation for our Contact Centres in Perth, Western Australia and Adelaide, South Australia. Our focus on supporting Silver Chain Group nationally to ensure the best possible outcome for every client has not changed. What has is our underlying capacity and capability.
The Silver Chain Group Board was formed in September 2011 and comprises members from both the Silver Chain and RDNS SA boards. The Board has extensive experience and knowledge in the health care industry, as well as commercial and business expertise which they bring to the Silver Chain Group.
The gift of a donation allows us to extend our services to meet a need in the community where there may be a shortfall in funding.

Dedicated volunteers have been supporting our clients and staff throughout our history.
Every year, many generous people leave a legacy to Silver Chain Group through Estates and Trusts. This year, we received a total donation of more than $2 million.
Silver Chain has Branch Committees in Western Australia that play an integral role in supporting the enhancement of client services and the up-skilling of staff via training and development opportunities.
Health Care East Coast has undergone significant changes over the past year in order to better prepare Silver Chain Group for upcoming expansion into new and exciting areas.
Health Care South Australia, which operates as RDNS SA, has had a year of consolidation and realignment, focussing on delivering high value integrated care for South Australians through the delivery of community health and aged care services.
Health Care Western Australia has continued to deliver sustainable community solutions across its range of core services and further strengthened the ‘system of care’ model across acute, complex and chronic, and end of life palliative care in the community.
