Expanding into Victoria
Silver Chain has expanded into Victoria with the commencement of the Western HealthLinks program in partnership with Western Health, a Melbourne Metropolitan Health Service.
Funded by the Victorian Department of Health and Human Services, Western HealthLinks is designed to help eligible patients living in the Western Health Metropolitan Health Service to manage their health from home, following an admission to hospital.
Silver Chain General Manager, East Coast, Clark Chambers, said through Western HealthLinks, patients will have access to a multi-disciplinary team, via a Health Navigator, who will develop an individualised care plan, based on the patient’s condition.
“The care plan will help determine what services will support the patient in managing their health in the community setting,” Clark explained.
“Patients and their family will have access to an online patient portal where they can view their care plan, scheduled visits from their Health Navigator and any appointments they may have with health care professionals and service providers.”
A small cohort of patients started to access the program from November last year. It is expected that over time the number of patients enrolled in the program will increase up to 2,500.
A team has now been recruited to support the program and a Silver Chain office has also been set up in the suburb of Sunshine in Melbourne. The team is expected to grow by June 2017 to include nurses and allied health professionals.
