Health Care East Coast

Silver Chain’s Health Care East Coast operations have continued to strengthen over the past financial year with the expansion of services and growth into new markets.

In November 2016, Silver Chain expanded into Victoria, providing health navigation and priority response services to clients in conjunction with Western Health as part of the Western HealthLinks program. The service is being run from our new facility in the western Melbourne suburb of Sunshine. Since commencement more than 1,180 clients have received services from Silver Chain.

Another highlight of the past year was confirmation that Silver Chain had been awarded a world-first Social Impact Investment to provide a comprehensive palliative care service to the residents of Western Sydney Local Health District (LHD). Much of the year was spent working in partnership with the New South Wales Office of Social Impact Investment, the New South Wales Ministry of Health and the Western Sydney LHDt to develop and launch the service.

Launched in June 2017, the Silver Chain Community Palliative Care Service will provide comprehensive palliative care services with 24/7 access to clinicians to more than 8,300 clients over seven years. To support our operations, Silver Chain has moved into an interim service delivery centre in the western Sydney suburb of Seven Hills with plans to move into a new facility in the new financial year.

Also in New South Wales, we continue to deliver the Last Days of Life palliative care program in three regional LHDs and in partnership with South Western Sydney LHD as the PEACH program in five metropolitan LHDs. Across the State we have delivered care to more than 1,300 clients as part of these important programs in the last year alone.

In Queensland, we continue to provide Hospital in the Home (HITH) services to patients in the Brisbane Metro North and Sunshine Coast Hospital and Health Services (HHS). In 2016/17 we provided HITH services to more than 1,400 clients across the two HHS.

Our corporate headquarters for the East Coast, Silver Chain House East Coast, located at the University of Wollongong’s Innovation Campus have been bolstered by the addition of a Management Team Assistant, Business Services Manager, Finance Business Partner, Business Development Partner and Quality and Safety Business Partner.

The team is looking forward to working on a number of exciting opportunities during the year ahead, to continue to support Silver Chain’s growth agenda through the provision of quality health care services across the eastern states of Australia.
