We are a not-for-profit organisation delivering community health and aged care services across Australia. We help people of all ages to receive the care they need so they can continue to live independently at home. We focus on individual strengths and abilities and offer our support and guidance where needed.

Silver Chain Group has had another exciting year which has included the further development of a number of programs aimed at building community capacity and challenging traditional approaches to service delivery.

Influence and Advocacy - Giving voice to our vision

The primary focus of Silver Chain Group’s Influence and Advocacy components of work has been based on a single principle - making an unprecedented impact on the health and wellbeing of all Australians.

The 2017/18 financial year was bittersweet for Access Care Network Australia as the program that started it all, the WA Assessment Framework, transitioned to My Aged Care.

Year in Review - Health Care

The 2017/18 financial year for the Health Care division has been one of growth and change.

Year in Review - Social Care

The Social Care division of Silver Chain Group has been delighted to support an ever-increasing number of people to live their best lives, within their homes and communities.

National Medical Director Update

The past year has demonstrated the continued maturation of Silver Chain Group as a health care organisation, culminating in the successful achievement of Australian Council of Healthcare Standards accreditation.

Our People

People are Silver Chain’s key asset. During the 2017/18 financial year, Silver Chain Group employed and onboarded 960 new staff members which took our overall headcount to 4,300 staff and over 300 volunteers, all of whom were vital in enabling us our vision; to deliver world’s best health and aged care.

Volunteers who give a little to change a lot

Volunteers make a huge difference at Silver Chain, including Lucia, who has been giving her time as a cook at the Fremantle Social Centre for an amazing 27 years.

hipages lends helping hand in Rockingham

A charitable community initiative by hipages has transformed Silver Chain’s Rockingham Social Centre’s backyard into a dementia friendly environment.

SA Government’s Domiciliary Care services takes on new life with RDNS SA

On 30 June 2018, RDNS SA became the new provider of all Domiciliary Care services (previously delivered by the SA Government). This change has quickly expanded the range of specialist health and home care support services offered by RDNS SA.

Integrum Aged Care+

An exciting development during the 2017-18 year has been the pre-pilot and pilot implementation of Integrum Aged Care +.

Thank you to the kindness and generosity of our loyal donors who have helped us raise more than $1.4 million over the past year.

Volunteer

Dedicated volunteers have been supporting our clients and staff throughout our history. Currently, our organisation benefits from the skills and experience of more than 280 volunteers who make a significant contribution, enhancing the quality of service we provide to clients and their families.
