Health Care South Australia
Health Care South Australia, which operates as RDNS SA, has had a year of consolidation and realignment, focussing on delivering high value integrated care for South Australians through the delivery of community health and aged care services.
The first Complex Wound Service was established in the northern suburbs of Adelaide providing advanced, specialised assessment and management for people with non-healing wounds. An evaluation of the service after six months of operation demonstrated a significant improvement in wound healing rates, with 70 per cent of clients either healed or healing. A second Complex Wound Service will soon be established in the southern suburbs of Adelaide.
The Virtual Hospital uses video technology to deliver nursing services remotely through broadband telecommunications. Recognising this is less obtrusive for clients and more efficient for delivering in-home services, the role of the Virtual Hospital has expanded to include SA Health clients under a community care service, palliative care syringe driver medication checks and paediatric calculation checks.
The Commonwealth Home Support Program (CHSP), an initiative of the Federal Government, was introduced in November 2015. The program provides an opportunity for clients to enter the aged care system through services with a strong focus on reablement and restorative care to help them rebuild their confidence and skills to carry out activities of daily living. With the introduction of CHSP, we reviewed the way we deliver services to ensure we have a strong focus on reablement and restorative care, and clients’ care plans are based on personal goal setting.
South Australia assumed operational responsibility for the Silver Chain Group’s national Alarms business with a focus on improving performance to provide an overall improved client experience. With renewed focus on Alarms, client calls are now being answered first time – an improvement of more than 40 per cent.
In response to the National Disability Insurance Scheme (NDIS), the Disability Training program – led by a highly skilled team of nurses who train support workers to provide care for clients with a disability – underwent a service model review. Services can now be tracked, costed and provided to clients at an individual level in line with NDIS requirements.
We endeavour to respond to client feedback and this year we focused on service provider continuity. We have embarked on reorganising the way services are delivered to clients to minimise disruption, where possible. Our staff will now be allocated to a team that reflects the service they provide rather than based on geography, to ensure greater consistency around which provider visits the client.
Over the coming year, we will seek to have further impact on the health system in South Australia in partnership with SA Health and other stakeholders.
