Silver Chain expands into Broome
In January 2017, Silver Chain expanded its health care services into the Kimberley region with the commencement of a Home and Community Care (HACC) nursing service in Broome.
Silver Chain’s service is co-located at the Broome Health Campus and supports eligible clients, once discharged, to access in-home or clinic based nursing services.
Silver Chain Director of Clinical Operations, Country WA, Sharon Hearns, said Silver Chain is excited to be providing HACC nursing services to the Broome community and looks forward to collaboration and partnership with local service providers in the delivery of care.
“Through the provision of this nursing service, a greater number of people in the Broome community will be able to access specialist community-based nursing care,” Sharon said.
“By supporting clients when they are discharged from hospital, through community-based care, we hope to see a reduction in potentially preventable Emergency Department presentations and hospital admissions.”
WA Country Health Service Kimberley Regional Director, Bec Smith, said she was pleased to welcome Silver Chain’s nursing service to the Broome community.
“The arrival of Silver Chain based out of the Broome Hospital heralds a significant improvement to the ability of our health services to provide out-of-hospital nursing services for those who are eligible under the HACC program," she said.
Silver Chain’s service is funded by the Department of Health enabling HACC eligible clients to access the service, via referral from their GP hospital of other health providers to Silver Chain.
