Health Care East Coast
An expansion of services in New South Wales has continued to strengthen Silver Chain’s Health operations on the East Coast.
In July 2017, Silver Chain’s Community Palliative Care Service, which provides our clients with 24/7 access to clinicians, began operations in the Western Sydney Local Health District (LHD).
More than 8,000 clients and their families will be supported by this service over the seven-year life of the project and in November 2017, we moved in to our permanent service delivery centre in Seven Hills, which is now home to more than 50 clinical and support staff.
Also in New South Wales, we continued to deliver the Last Days of Life palliative care program in three regional LHDs and in partnership with South Western Sydney LHD as the PEACH program in four metropolitan LHDs. Across the state we have delivered care to more than 1,030 clients as part of these important programs in the last year alone.
In Queensland, we continue to provide Hospital in the Home (HITH) services to patients in the Sunshine Coast and Brisbane Metro North Hospital and Health Services (HHS).
This financial year, we provided HITH services to more than 630 clients across the two HHS’.
In Victoria, the Western HealthLinks program is providing health navigation and priority response services to clients in conjunction with our partners in Western Health. Since commencement, more than 3,000 clients have been referred to our team based in Sunshine.
The East Coast team is looking forward to working on a number of exciting opportunities during the year ahead, and to continue to support Silver Chain’s growth agenda through the provision of quality health care services across the eastern states of Australia.
